South Dakota's coverage picture changed meaningfully when voters approved Medicaid expansion, which took effect in 2023 — the state's lowest earners now have an option they didn't before. For everyone else, especially the ranchers, contractors, and small business owners who define the state, the job is still the same: find real coverage at a price that makes sense.

South Dakota uses the federal Marketplace at HealthCare.gov. With Medicaid expansion in effect, adults earning up to 138% of the federal poverty level qualify for Medicaid — no coverage gap.
Above that line, premium tax credits are still available, but the enhanced subsidies expired at the end of 2025, and South Dakota's Marketplace has a small number of carriers.
